Phase II Decision-Making Map

This resource outlines all of the significant decisions that need to be made throughout Phase II and provides guidance for a decision-making structure. If needed, review the Implementation Support Team Role and Responsibilities and Implementation Support Team Leader Roles and Responsibilities documents for information about the team.

Agenda for Phase II Launch Meeting

This resource is a sample meeting agenda for the launch of the Implementation Support Team. During the meeting, the team defines team norms, roles and responsibilities, decision-making, and creates the team charter.

Stakeholder Feedback Survey

This resource provides sample survey questions to gather feedback from teachers, students, and leaders.

Selection Pacing Schedule

This resource provides a sample pacing schedule for schools, districts, or charter networks to use as a starting point for the selection process. This includes both 20-week (5-month) and 40-week (10-month) schedules.

Implementation Plan

This resource is a template for keeping track of all of the projects and tasks that your team will take on during implementation. Before the work plan, there is space for recording major decisions and notes relevant to your work plan from across Phase I and Phase II: vision, core beliefs, goals, communications, roles and responsibilities, and decision-making processes. This is meant to be a “one-stop shop,” but the format may not be right for your team. Modify as needed—add or delete sections, adjust tables and rows, or separate into multiple templates.

Guidance for Observing Implementation

This resource is a list of the key interactions that early implementers benefitted from
observing in order to understand how things were going. Some early implementers
created shared documents to pool observations and impressions.
